The Wound Care Nurse, in collaboration with the physician and physical therapy, coordinates quality, cost effective care for the patients and their families who choose Casa Colina for their medical and rehabilitation needs. The components of the Wound Care Nurse duties will include: direct patient care, education, consultation, counseling and case management. The Wound Care Nurse develops a plan of care with the physician, patient, and the patient's family from admission to discharge or continuum of care. The Wound Care Nurse coordinates, monitors and in collaboration with physician redirects wound care as necessary to maximize the patient's outcomes within the appropriate length of stay, through efficient use of resources and within established standards.
Requirements: Registered professional nurse holding a current and valid license to practice nursing in the State of California. Bachelor of Science degree with major emphasis in Nursing preferred.
