Position Summary: The Member Liaison /Advocate acts as an advocate for high-risk members and helps patients understand and navigate the managed care system. The purpose of the Member Liaison/Advocate role is to lower re-admission rates by: visiting members after they have returned home from the hospital to connect them to their interdisciplinary care team, collect valuable information, provide linkages to necessary resources and provide quality non-clinical services to members to facilitate their transition home.
Essential Duties & Responsibilities include the following : - Conduct field-based visits to high-risk members during and following hospitalizations.
- Educates members on navigating services within managed care and connects members to the resources they need internal and external to the medical group and the Member Advocate Program.
- Maintains strong working knowledge of managed care and healthcare systems.
- Build and maintain positive relationships with members to facilitate care management.
- Connects the member to their assigned nurse case manager and pharmacist, as appropriate.
- Presents and reviews pertinent health related information with member, including post-discharge instructions, urgent care options, and benefits information for members.
- Assists assigned members with non-clinical post-discharge needs.
- Assist patients with scheduling medical appointments and non-emergency medical transportation.
- Maintains routine communications with members providing timely follow-up to ensure resolution of issues.
- Provides clear and complete documentation of all visits.
- Accurately captures information while in the member's home and convey pertinent information to the Regal care team.
- Administer basic, non-clinical assessments.
- Conduct visits and member follow-up in adherence with Member Advocate Program guidelines, policies and protocols.
- Supports complex members through ongoing support and visits in accordance with department protocols
- Serves as a non-clinical resource for both members and internal stakeholders
- Serve as a departmental resource for program-related needs and questions
- All other duties as directed by management.
The pay range for this position at commencement of employment is expected to be between $30 per hour to $32 per hour Dependent on Experience; pending on multiple individualized factors, including market location, job-related knowledge, licensure, skills, and experience.
The total compensation package for this position may also include other elements, including a sign-on bonus and discretionary awards in addition to a full range of medical, financial, and/or other benefits (including 401(k) eligibility and various paid time off benefits, such as vacation, sick time, and parental leave), dependent on the position offered.

Education and/or Experience: - High School Diploma (required)
- Bachelor's degree (strongly preferred)
- Minimum 2-3 years' experience in healthcare or social service sectors
- Excellent verbal and written communication skills in English
- Proficient at Microsoft Office products
- Ability to work independently in the field, while still meeting departmental deadlines and requirements
- Bi-lingual (Spanish or Armenian) strongly preferred
- Valid driver's license required
- Good Driving record preferred
